Doelgroep

Deze 2-daagse Engelstalige opleiding is gericht op fysiotherapeuten die hun musculoskeletale echografie vaardigheden willen verbeteren. De nadruk ligt op dynamische beoordeling en 'muscle retraining' en niet op de diagnostiek.

Doel van de opleiding

Aan de hand van presentaties, e-learning, demonstraties en praktisch oefenen word jij wegwijs gemaakt in de rehabilitative ultrasound imaging. De Australische dr. Alison Grimaldi zal deze opleiding in het Engels verzorgen. Er zijn Nederlandse docenten van het NT-e aanwezig om haar te assisteren.

How will RUSI help you optimise rehabilitation outcomes?

While isolated deep muscle training is not a panacea for musculoskeletal pain and does not alone provide functional alignment control, there are many reasons why real-time ultrasound assessment and retraining remain valuable tools in the management of musculoskeletal pain and optimisation of physical performance.

  • RUSI allows assessment of muscle size and quality (echogenicity) that is not possible through visual inspection or palpation.
  • RUSI allows access to deeper musculature that is often not accessible via direct palpation.
  • RUSI allows assessment of motor control. While strength is extremely important, many patients and athletes fail to progress under a strength-only regime. Many patients with ongoing pain states and athletes failing to progress in their sporting pursuits have developed inefficient muscle recruitment strategies. EMG studies for many different pain issues are now telling a similar story – muscle recruitment strategies become less refined, a whole muscle synergy recruits together with excessive levels of co-contraction, instead of individual muscles and muscle segments playing their individual tunes amongst the orchestra of finely controlled and efficient movement. Excessive co-contraction is thought to contribute to overload of the muscles themselves, as well as of the underlying joint structures or tendons and nerves that pass under or through the muscles. This situation can become painful and tiring. After all, we have evolved as very efficient beings so that when functioning optimally we only recruit the muscles required, and at an intensity that is required, to complete a task. RUSI is a powerful tool in identifying and retraining motor control dysfunction.
  • RUSI is useful in both upregulation and downregulation of muscle recruitment. In the patient with deconditioned or inhibited musculature, RUSI can be used to upregulate target muscles. On the other end of the spectrum, RUSI also provides excellent feedback when trying to downtrain excessively active musculature.
  • Seeing it is more powerful than feeling it, in early recruitment/movement retraining – for both the client and the therapist. Patients in pain often have poor body awareness in their region of pain, thought to be linked with smudging of the homunculus. Asking patients to do an exercise that requires fine motor control can be very challenging if their kinaesthetic awareness is poor. This often results in a disheartened patient and poor exercise compliance. Being able to see fine movements on the ultrasound screen provides a huge boost to confidence and compliance. Visual feedback can be gradually reduced as kinaesthetic awareness improves. For the therapist, once you start using RUSI you will realise that often what you thought you were feeling, or seeing externally, in not exactly what is happening internally. RUSI allows much more specific assessment and the ability to provide much more specific feedback.
  • Create neuroplastic change. Changes in both the sensory and motor homunculus have been demonstrated in those with ongoing pain states. Exercise that involves focussed attention to the body part is able to impart neuroplastic change that does not appear to occur with general exercise.

Neck and shoulder:

  • Imaging the scapular stabilisers: lower & upper trapezius and serratus anterior in low & higher load tasks
  • Rotator cuff assessment & retraining
  • Dynamic assessment of subacromial impingement & immediate biofeedback to improve
  • Cervical extensor assessment & retraining

Knee, foot, ankle:

  • Vastus medialis retraining
  • Popliteus retraining for posterolateral corner pain & instability
  • Assessment & retraining of the foot intrinsics
  • Bed based and more dynamic functional imaging will be explored

Lesdag 2

Hip:

  • Assessment & retraining of the hip flexors
  • Assessment & retraining of the hip abductors
  • Assessment & retraining of the deep external rotators
  • Bed based and more dynamic functional imaging will be explored

Gluteal tendinopathy:

  • Tendinopathies of the hip and pelvis represent a large burden on both the sporting and ageing populations. Growing evidence is shaping contemporary conservative management of tendinopathy. This workshop provides participants with guidance towards a deeper understanding of gluteal tendinopathy and more effective clinical management. The information presented is based on both an emerging scientific evidence base and over 25 years of clinical experience. Recent literature has advanced our understanding of mechanobiological mechanisms, risk factors, impairments and evidence regarding the relative efficacy of clinical tests and management approaches.

De opleiding bestaat uit 2 lesdagen, van 9.00u tot 18.00u.
Een lekkere lunch is inbegrepen.

  • E-learning: Dynamic Stabilisation of the Foot & Ankle, providing an in-depth review or foot and ankle functional anatomy. Powerpoint lectures with audio and handouts are presented online and are available for access via any device, at any time. Stop, rewind, log out and in as many times as you like over a 3-month access period.
